The Shawn Hornbeck Foundation offers many programs for the public. We offer our Kidz Kardz Child ID cards as well as our Making Trax Abduction Safety program. If you would like to have the Shawn Hornbeck Foundation attend your next event please fill out the following form and SUBMIT it and a representative will contact you with additional information. ALL items in RED are Required for your form to be accepted and reviewed by the Shawn Hornbeck Foundation for consideration.

We do provide our programs for FREE for families, but donations are GREATLY APPRECIATED as we use the donations to help us continue our programs for FREE to as many families as possible!
